Thank you for participating in this good-humored fundraiser! If you would like to send the Prom Potty to a friend's yard, please see the RULES section (in bold print) at the bottom of this description before completing this form.
- If the Prom Potty is in your yard NOW, and you wish to to have it removed, select "Removal-- $25" (suggested donation). If you would like to donate a different amount, select "other" in Section 3 of this form.
- If you DON'T have the Prom Potty in your yard but wish to send it to a friend, please select "Delivery- $30."
- If you have the Prom Potty in your yard and want to have it removed AND want to pass it on to a friend, select "Removal AND Delivery- $50."
- If you'd like to prevent the Prom Potty from showing up in YOUR yard (or returning to your yard), select "Insurance- $50" to safeguard your yard from a Prom Potty invasion. :)

RULES:
- FOR THE SAFETY OF OUR VOLUNTEERS, PLEASE ONLY SELECT RECIPIENTS/LOCATIONS THAT YOU FEEL CONFIDENT WILL BE RECEPTIVE TO THIS FUN APPROACH TO FUNDRAISING.
- YOU WILL BE ASKED TO PROVIDE A PHONE NUMBER (IN ADDITION TO THE ADDRESS) FOR THE RECIPIENT, TO ENSURE THAT THE RECIPIENT IS SOMEONE THAT YOU PERSONALLY KNOW.
- THE POTTY CANNOT BE DELIVERED MORE THAN 15 MILES FROM LAVACA HIGH SCHOOL.
WE WANT THE POTTY TO KEEP MOVING, SO KEEP THAT IN MIND WHEN SENDING IT ALONG-- THE GOAL IS TO RAISE AS MUCH MONEY AS POSSIBLE; IN ORDER TO DO THAT, THE POTTY MUST MOVE. :)
Disclaimers: toilet is not for actual usage---no plumbing is present; the toilet has been thoroughly sanitized.
